Children's Case Management
Children and youth let us know in many different ways that they are in crisis and need extra care. At University Settlement young people with complex mental health issues get the help they need through two related programs.
BLENDED CASE MANAGEMENT
The Blended Case Management program provides intensive and supportive case management services to emotionally challenged youth between the ages of 5 and 21 to prevent emergency room visits, psychiatric hospitalizations and placement in residential treatment facilities. Through home visits, families are provided with ongoing assistance and advocacy in order to access mental health, education and income support programs that are essential to meeting their child's needs. Services can be short- or long-term, depending on the needs of the family.
Our case managers are trained and certified in case management by the New York State Department of Health and Mental Hygiene. They are also distinguished by their expertise in working with mentally ill children who have experienced sexual abuse.
Services are available to families through Manhattan, and are provided in English, Spanish, Cantonese and Mandarin.
HOME BASED CRISIS INTERVENTION
The Home Based Crisis Intervention program provides intensive in-home case management services for emotionally disturbed youth ages 5 to 17 and their families free of charge. Professionally trained case managers intervene when a child or adolescent is at imminent risk of hospitalization or removal from their home due to a psychiatric crisis.
Working with a family for a period of six to eight weeks, the case managers use a team approach to stabilize the immediate crisis and set up long-term support services to manage the child's mental health needs and help keep the family together. By working together with the family and other service providers or community-based programs, our case managers can ensure the coordination and continuity of quality care within each client's community.
Services are available in English, Spanish, Cantonese and, as of this year, French.
Our case managers are trained and certified by the New York State Department of Health and Mental Hygiene.
The Home Based Crisis Intervention program is funded through the NYS Office of Mental Health and the NYC Department of Health and Mental Hygiene.
