Family Team Conference Facilitator

The Prevention Program provides comprehensive home and community-based preventive services to children and their families. The program will provide services supported by the evidence-based model, Mobility Mentoring. The Program model includes working from a coaching model wherein the interdisciplinary team works closely with families in the community to establish and accomplish family-centered goals. As the Family Team Conference Facilitator, you will serve as an integral member of the Prevention team ensuring the core principles and practices of Mobility Mentoring are integrated into the work and planning with all families. You will support and ensure the stability and growth of families based on their identified goals and plans and ensure children and families have access to resources and services that help reach their optimum goals.


What You Will Be Doing:

  • Facilitate FTCs to review services, assess progress together, and help develop a plan of action
  • Utilize the six-stage model for FTC and encourage and support the use of Mobility Mentoring
  • Maintain an active and flexible FTC schedule that meets the needs of families and compliance guidelines of ACS
  • Serve as an active member of the interdisciplinary team
  • Conduct visits to participant homes in order to hold conferences when needed
  • Submit accurate and timely documentation as required by ACS in PROMIS and Connections databases
  • Conduct follow-up reviews
  • Facilitate ongoing and/or short-term participant groups or workshops
  • Attend and participate in all meetings

What We Are Looking for:

  • You have a Master's degree in Social Work or related field.
  • You have 2+ years of experience in a child welfare-related setting.
  • You have strong communications skills and have excellent group facilitation skills.
  • You thrive in a busy environment and able to shift hears when needed.
  • You are proficient in Microsoft Office and familiar with Connections and PROMIS databases
  • You are willing to be an advocate for your program, participants and community.
  • You have a strong commitment to advancing diversity, equity, and inclusion.
