Rental Spaces @ the Houston Street CenterThe following is a description of the forms that must be completed. Space Usage Application: This is the first form that must be filled out by new renters. It asks for general information about your organization. After it is returned to us, it will be reviewed by the Community Center Director. If approved, then the remaining forms below will need to be completed. Rental Agreement: This document contains the details of the relationship and expectations. This form is only filled out once a year or when there is a significant policy change. Please read this carefully and call with any questions. Rental Contract: This form is to be completed with every rental request. For example: if you are requesting space for an 8 week course you only need to fill out this form once. Upon receipt, we will review the rental contract, check for availability and confirm with you when it has been approved. Space Set-Up Form: Use this form to specify your set-up needs.
