Overview:

The Blended ICM/SCM Model is an innovative service/treatment program, primarily intended to maintain children who have a mental illness and assist them in gaining access to needed mental health, medical, educational, social, recreational and other services essential to meeting basic human needs. The target population is children and adolescents (age 5-18) at risk of psychiatric hospitalization and/or residential placement.

Responsibilities:
  • Provide a minimum of four home visits to each family a month.
  • Conduct outreach to client and family.
  • Assess client and develop service plan.
  • Facilitate service delivery.
  • Coordinate treatment plans with service providers.
  • Maintain complete case management records.
  • Provide crisis intervention, prevention and resolution as required.
  • In conjunction with the Program Director, he/she will:
  • Be responsible for team supervision
  • Conduct regular chart reviews to assure the delivery of quality services and supported by documentation.
  • Review new intake referrals and assign new cases to case managers.
  • Coordinate trips and programs operated by the department.
  • Represent the program and the agency at external meetings.
  • Assist the Director in overseeing the wrap around budget of the program.
  • Other duties as assigned by the Program Director
Qualifications:
  • MA in Social Work or a related discipline, leading to a thorough understanding of child psychiatric treatment is required.
  • Two years of supervisory experience, at least four years of providing case management services to children
  • Demonstrated ability to engage hard-to-reach populations within their communities is crucial
  • Ability to participate as a team player is a must.
  • Good verbal and written skills required.
  • Staff should reflect the ethnic/ racial populations of the neighborhoods being served (i.e. Lower Manhattan, Harlem and Washington Heights).
  • Bilingual in Chinese or Spanish preferred.
Salary:

Commensurate with experience

Hours:

Full-time (35 hours) Start Date: July 08
